Welcome to the Academy of Osseointegration electronic evaluation program.

This year, the evaluation process for attendees of the 2013 Annual Meeting is electronic. All AO session evaluations and the overall meeting evaluation will be completed on-line. Please complete Evaluations as thoroughly as possible. We are counting on your input to help make next year's conference the best it can be!

What you need to know:

• You will be asked to enter your LAST NAME and ID Number in order to complete the evaluations.
Your ID Number is located on your registration card and badge.

• The Evaluation site will be live through May 24, 2013. You will be able to complete the evaluations from your home or office
computers through that date.
